Company: Forge Medical Staffing, LLC · Georgia (“Forge,” “we,” “us”) provides locum tenens / contract anesthesia staffing and related operations tools for clinicians and facilities.
Forge provides a locum scheduling app for invited clinicians. It is not a clinical product, it does not give medical advice, and it is not a medical device.
This is the privacy policy for the Forge Medical Staffing locum product: the locum site at https://app.forgemedstaffing.com, the iOS app, and the Android app. It covers the account, schedule, Vault, hours, camera, and credential information described below.
If you have an invite, we store the name, email, phone, role, schedule, hours, and credential files you or Forge put on your account. Vault uploads may include licenses, ID photos, or optional TB and vaccine records you choose to send. When a hospital reviews your time, your name, credential or specialty, and hours go to that hospital's contact for approval (and appear on the weekly roster when that hospital is opted in). We do not sell that data. We do not sell phone numbers. Facility bill rates are never shown to locums.
The iOS and Android apps are Capacitor clients. They include a native tab bar, show your account in an in-app WebView, and request camera access when you add a Vault credential or a travel receipt.
They are invite-only staffing tools for clinicians. The apps open the locum site. An admin who signs in is sent to the admin operations site (admin.forgemedstaffing.com), which may stay inside the same WebView.
The app asks for camera or photo-library access when you upload a credential to Vault, or a travel receipt on Timesheets when that hospital allows travel expenses. Vault credentials may include licenses or ID photos you choose to send. There are no in-app purchases.
Open Settings → Delete my account and type DELETE to confirm. That closes your login, marks the person inactive, and removes your name, email, and phone from the live record. Hours or pay history Forge must keep for records stays without your contact details. Vault files you uploaded may also be retained for staffing and compliance records.
Can't sign in, or don't have the app? Email hello@forgemedstaffing.com from the address on your account and ask us to delete it. You can also email that address to request deletion of specific data without closing your account.
Locums never see facility bill rates. Bill rates and Forge Pay / Money tools stay with Forge staff on the admin operations site.
Forge is non-clinical. It does not diagnose, treat, cure, or monitor patients. It is not a medical device and it is not medical advice.
Sign-in uses a session cookie on your device so you can stay signed in. The apps do not include a third-party advertising SDK and do not use an advertising identifier.
We may use limited device and diagnostic details (such as operating system, app version, and error information when sign-in or an upload fails) only to operate the app and fix failures. We do not sell that information.
The apps are rated 17+ and are not for children. Forge is for invited clinicians. We do not knowingly collect personal information from anyone under 17. If you believe we have, email hello@forgemedstaffing.com and we will delete it.
We keep account, schedule, Vault, and hours records for as long as needed to run staffing and meet legal duties. After you delete your account, contact details come off the live record as described under Account deletion.
We retain messaging consent and related logs as needed for legal, compliance, and operational purposes, then delete or de-identify when no longer required.
This section describes how we collect, use, and protect information in connection with our SMS / text messaging program used for operational staffing communications (for example: account invites, weekly schedules, and clock-in / clock-out reminders). Forge Medical Staffing sends transactional SMS only: account invites, weekly schedules, and clock-in / clock-out reminders. This is a Forge-only program. We do not send marketing or promotional spam, and we do not send affiliate, third-party, or lead-generation texts.
You opt in only on the public web form at https://forgemedstaffing.com/sms-opt-in, which is linked from https://forgemedstaffing.com/. Name is optional. Mobile number is required. You must check an unchecked required consent checkbox, then submit. The consent text covers transactional SMS about account invites, weekly schedules, and clock-in/out; message frequency varies; message and data rates may apply; reply STOP to cancel and HELP for help; consent is not a condition of employment or staffing work; and it links to this Privacy Policy and the SMS Terms. The form POSTs to https://forgemedstaffing.com/api/opt-in. Forge stores your consent and sends a confirmation SMS. The public web form at https://forgemedstaffing.com/sms-opt-in is the only documented opt-in path for this SMS program. Consent is given directly to Forge, is optional, and is not a condition of work.
Message frequency: Message frequency varies. You may receive several messages per week depending on your assignments and schedule changes.

We do not share, sell, or provide your mobile phone number or messaging consent data to third parties or affiliates for marketing or promotional purposes.
Forge does not share mobile phone numbers or messaging consent with partners, affiliates, or third parties for marketing or promotional purposes. We do not sell or buy consumer opt-in lists.
A messaging delivery processor (our SMS platform) may send program messages you opted into. That processor may not use your mobile number or messaging consent for its own marketing or promotional purposes. This is delivery processing only — not affiliate, partner, or lead-generation sharing.
Text messaging originator opt-in data and consent will not be shared with any third parties. This excludes mobile opt-in information from being sold, rented, or shared with affiliates or third parties for their marketing or promotional purposes.
We may update this policy. The effective date above will change when we do. Continued use of the locum app, the locum site, or the SMS program after an update means you accept the revised policy.
